The Care Management Support Assistant contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Management Support Assistant performs advanced administrative/operational/customer support duties that require independent initiative and judgment. May apply intermediate mathematical skills.

Responsibilities

The Care Management Support Assistant performs calls (outbound) done by an Auto Dialer or manually as well as receives (inbound) calls from CarePlus members. Performs a Health Risk Assessment to our CarePlus Members.

  • Manage Inbound and outbound calls for HRA (Health Risk Assessment) effort
  • When needed, research for new telephone numbers by calling PCP offices and/or Pharmacy
  • The hours for this position are 8:30 PM-5:30 PM Monday-Friday during the months of March to November & November to February 9:30 PM-6:30 PM due to Daylight Savings Time.
  • Occasional overtime and weekends may be required
  • Must be comfortable taking inbound and outbound calls continuously

SCHEDULE: The hours for this position are 8:30 PM-5:30 PM Monday-Friday during the months of March to November & November to February 9:30 PM-6:30 PM due to Daylight Savings Time Occasional overtime and weekends may be required.

Required Qualifications

  • Fully Bilingual Spanish/English
  • Language assessments will be required as part of the interview process
  • Working knowledge of Microsoft Office Word, Excel, Outlook
  • Administration support experience in a healthcare industry with familiarity with care and well-being resources

  • To ensure Home or Hybrid Home/Office associates’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office associates must meet the following criteria:
  • At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended to support Humana applications, per associate.
  • Wireless, Wired Cable or DSL connection is suggested.
  • Satellite, cellular and microwave connection can be used only if they provide an optimal connection for associates. The use of these methods must be approved by leadership. (See Wireless, Wired Cable or DSL Connection in Exceptions, Section 7.0 in this policy.)
  • Humana will not pay for or reimburse Home or Hybrid Home/Office associates for any portion of the cost of their self-provided internet service, with the exception of associates who live or work from Home in the state of California, Illinois, Montana, or South Dakota. Associates who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.
  • Humana will provide Home or Hybrid Home/Office associates with telephone equipment appropriate to meet the business requirements for their position/job.
